With less than a month to practices beginning for the 2011-2012 season, it’s time to get some paperwork in order and get ready for fall.

Here we go!

Player information

If you haven’t already, please send these documents now to your team manager:

• Copy of your birth certificate
• USA Hockey membership registration. Your team manager will use this to scan and put you on the roster, so you’ll need to send the copy with a barcode, not just the number. If you haven’t registered yet, go to www.usahockeyregistration.com If you have already registered and just need a duplicate copy, go to https://www.usahockeyregistration.com/duplicate_request_input.action and put in player last name, birth date, and zip code. A confirmation and member number will be sent to the email used when registering online. You can either print and give your manager a copy, or email the file.

• USA Consent to Treat/Medical Form

imPACT testing August 20 and August 27

Baseline testing is set for August 20 and August 27 from 8 a.m. to 2 p.m. at Texas Health Plano Presbyterian http://www.texashealth.org/landing.cfm?id=117

• If you haven’t already, send to your team manager now the date and time you will be going to testing.
• Please take $10 cash or check payable to Texas Health. This covers the cost of baseline testing. All other program costs are covered by the Ice Jets.
• If you were tested before the 2010-2011 season for AT&T Metroplex High School League participation, you do not need to take another baseline test. Please, however, send to your team manager that testing has been completed so we have a record on file.

Apparel Day August 27
